Host Home Provider  

This residential option provides home and support services by a host home/companion care provider. 

 The host home/companion care provider assists the individual with direct personal assistance with activities of daily living and with facilitating inclusion in community activities, use of natural supports, social interaction, participation in leisure activities and supervision of the individuals’ safety and security. 

 Direct Support Professional 

A DSP cares for individuals experiencing developmental or intellectual disabilities or illnesses.  

Their main duties include helping patients complete basic housekeeping tasks, transporting patients to appointments or other social outings and keeping patients safe from potential health hazards in their surrounding environment. 

Community Supports Professional 
 

Formerly titled PAS/HAB

Duties consist of but are not limited to providing planning, organizing and providing community support services to adults with intellectual and developmental disabilities within the community. Teaching adults with intellectual and developmental disabilities independent living skills within their community environment. This position also provides transportation services for clients. Must be willing to travel and work a flexible schedule.

ISS Skills Mentor

The Skills Mentor works with clients through weekdays both at our facility and in the community. Responsible for the daily supervision, care training, assessment and/or assistance of individuals with Intellectual and Developmental Disabilities (IDD). Monitor and direct consumers in and outside of our facility. Contributes to and promotes the general well-being of clients in the Day Habilitation through the teaching of daily living skills, personal and social adjustments skills and job readiness training. Provide clients with sensory stimulation and fine and gross motor skills maintenance and improvement. Assist and provide transportation to individuals according to their service plan. Must be able to make independent decisions while working with consumers and keep immediate supervisor informed of ALL consumer and contract needs. This is an hourly position with no guarantee of set number of hours.
