HCS Services

The HCS Program provides services to people with IDD who live with their family, in their own home, in a foster/ companion care setting, or in a residence with no more than four people who also receive services. The HCS program provides services to meet the person’s needs so the person can maintain himself or herself in the community and have opportunities to participate as a citizen to the maximum extent possible. Service coordination is provided by their Texas Local Authority (LIDDA). Residents of foster/companion care setting or an HCS resident pay for their own room and board with either their monthly Supplemental Security Income check or other personal resources.

HOST HOME
COMPANION CARE

Habilitation services are provided to individuals of all ages in home-like environment as an alternative to living in a group home.  Individuals are responsible for room and board.

SUPERVISED LIVING

Provides habilitation, supervision and availability of a service provider staff to help the individual during normal sleeping hours in a group home setting. Individuals are responsible for room and board.

INDIVIDUALIZED SKILLS AND SOCIALIZATION (ISS)

Services provide the opportunity to participate in day habilitation for up to six hours a day, five days a week. These services are designed to help individuals learn, retain and improve self-help, socialization and adaptive skills necessary to live successfully in the community.

NURSING

Services provided by a licensed nurse include monitoring health, medication administration and monitoring, referrals for medical services, and training of individuals/family and support personnel.

SUPPORTED EMPLOYMENT

Provides assistance to sustain paid employment for individuals for whom, because of their disability, require intensive on-going support to perform in a work setting. Supported employment must be provided in work sites in which people without disabilities are also employed. Services include adaptations, supervision, and training as related to the individual’s diagnosis. This service can be provided in conjunction with day habilitation with a limit of 150 units a year.

MINOR HOME MODIFICATIONS

Services are designed to assess the need for, arrange for, and provide modifications and/or improvements to an individual’s home to allow for community living and ensure safety, security, and accessibility. The lifetime maximum amount for MHM is $7,500, with an annual allotment for maintenance and repairs of $300 per service plan year.

RESIDENTIAL SUPPORT

Services provide supervision and assistance from a service provider who is awake during normal sleeping hours in a group home setting.  Most individuals who receive residential support services do so to address needs such as special medical conditions, maladaptive behaviors and assistance with activities of daily living.  Individuals are responsible for room and board.

BEHAVIORAL SUPPORT

Provides for a licensed psychologist, psychological associate, DADS certified psychologist or a board certified behavioral analyst to provide specialized interventions that help individuals increase adaptive behaviors to replace or modify maladaptive or socially unacceptable behaviors that prevent or interfere with the individuals inclusion in home and family life or community life.

OCCUPATIONAL THERAPY

Services are designed for skilled treatment that will help individuals achieve independent functioning in all aspects of their lives.  Services include assessments, development of treatment plans, therapeutic intervention, training, assistance with adaptive aids, and consultation with family/provider.

ADAPTIVE AIDS

These are necessary medical supplies, devices, controls or appliances that allow individuals to retain or to increase their abilities to perform activities of daily living or control their environment.  This service provides adaptive aids necessary to treat, rehabilitate, prevent or compensate for conditions resulting in disability or loss of functioning.  Annual allocation is $10,000 per service plan year.

RESPITE

Provides short-term services to individuals who cannot care for themselves; furnished because of the absence of or need for relief of the primary care giver. Respite services are provided both in home and out of home and are limited based on the plan.

SUPPORTED HOME LIVING

Provides assistance to individuals living in their own or family homes. Services include assistance with learning, retaining, or improving skills related to activities of daily living such as personal grooming and cleanliness, bed making and household chores, preparing and eating food, and the social and adaptive skills necessary to allow individuals to live in their own homes in the community.

PHYSICAL THERAPY

Services are designed to help individuals with range of motion, improve physical functioning, and retain physical conditioning. Services include assessments, development of treatment plans, therapeutic intervention, training, assistance with adaptive aids, and consultation with family/provider.

SPEECH THERAPY

Services are designed for corrective or rehabilitive treatment of individuals with speech or language disorders. Services include assessments, development of treament plans, therapeutic intervention, training, assistance with adaptive aids, and consultation with family/provider.

AUDIOLOGY

Provides for a licensed audiologist to perform assessments and direct therapeutic intervention, training and consultation with family/provider.

DENTAL

Individuals 21 and older:$2,000 per IPC year to address emergency, preventive and therapeutic dental treatment. Includes routine examinations.

DIETARY

Provides for a licensed dietician to perform face-to-face assessment, consultation and staff training. Services include basic and/or special therapeutic nutritional needs.

SOCIAL WORK

Assessment and direct therapeutic intervention. Training and consultation with family/provider.
